ZORYVE Improved Sex Lives in Genital Psoriasis Trials
ZORYVE (roflumilast) beat vehicle on sexual difficulties, itch and quality of life in genital psoriasis subgroups across three phase 3 trials, 2026 conference data show.
Counter notes
- 39.3 percent of genital psoriasis patients on ZORYVE cream reached treatment success at week 8 versus 5.4 percent on placebo.
- 71 percent of genital psoriasis patients on the cream saw meaningful itch reduction versus 16.7 percent on placebo.
- Analyses covered DERMIS-1, DERMIS-2 and ARRECTOR phase 3 trials; data presented at the 2026 Fall Clinical Dermatology Conference in Las Vegas.
- Genital involvement at baseline: 15.6 percent of cream trial participants and 17.4 percent of foam trial participants.
- Fewer than 1 percent reported stinging with definite discomfort 10-15 minutes after first application.

Nearly 40 percent of genital psoriasis patients reached treatment success with ZORYVE cream 0.3 percent in eight weeks, against 5.4 percent on placebo. New analyses presented at the 2026 Fall Clinical Dermatology Conference in Las Vegas, reported by Dermatology Times, found the steroid-free roflumilast also reduced patient-reported sexual difficulties and relationship problems linked to psoriasis compared with vehicle.
The numbers matter because genital psoriasis is common and undertreated. Higher-potency steroid creams are generally not recommended for routine use on such sensitive areas.
What did the trials measure?
The analyses drew on three phase 3 trials: DERMIS-1 and DERMIS-2 tested ZORYVE cream 0.3 percent in plaque psoriasis. ARRECTOR tested ZORYVE foam 0.3 percent in scalp and body psoriasis. Roughly one in six participants had genital involvement at baseline: 15.6 percent in the cream trials and 17.4 percent in the foam trial.
Participants rated how much psoriasis caused sexual difficulties and problems with partners, close friends or relatives. After eight weeks, both measures favored ZORYVE over vehicle — overall and in the genital psoriasis subgroup.
How did it perform on clear skin and itch?
The efficacy gap was wide.
- Cream, all patients: 39.9 percent treatment success versus 6.5 percent on placebo at week 8.
- Cream, genital subgroup: 39.3 percent versus 5.4 percent.
- Foam, genital subgroup: 45.1 percent versus 5.7 percent.
Itch relief followed the same pattern, though the figures reflect overall severity rather than genital symptoms specifically. Among genital psoriasis patients, 71 percent on the cream saw a meaningful itch reduction versus 16.7 percent on placebo; the foam posted 63.6 percent versus 37.9 percent.
Quality-of-life gains were substantial. In the genital subgroup, 64.4 percent on the cream and 81.8 percent on the foam reported meaningful improvement, against 34.6 and 33.3 percent on placebo.
Why does the steroid-free formulation matter?
Long-term topical corticosteroids can thin the skin, so clinicians prefer lower-potency options for the genitals. ZORYVE takes another route. It is a PDE4 inhibitor that reduces inflammation, formulated without ethanol, propylene glycol or fragrance. That composition is closer to a moisturizer's discipline than a classic steroid's brute force.
Tolerability held up. Fewer than 1 percent of participants reported a hot, tingling or stinging feeling with definite discomfort 10 to 15 minutes after first application. Both formulas were generally well tolerated, including in the genital psoriasis groups.
What are the limits?
The genital psoriasis subgroups were small. The analyses covered only eight weeks, and most participants were white. The study authors suggest dermatologists consider raising sexual function with genital psoriasis patients rather than waiting to be asked.
For a category long treated with caution and half-spoken appointments, the data give clinicians a concrete reason to start that conversation — and a steroid-free option to back it up.
