"Vulvar deflation is a common yet under-recognized consequence of significant weight loss," says Juan Quintero, MD, a board-certified plastic surgeon in Louisville, Kentucky. As GLP-1 medications reshape obesity treatment, both plastic surgeons and gynecologic specialists report growing numbers of women seeking solutions for changes in the appearance and function of the vulvar region.

The phenomenon, colloquially labeled "Ozempic Vulva" or "Ozempic Vagina" on social media, describes the physical and aesthetic effects of rapid weight loss on female genitalia. "Vulvar deflation refers to the loss of soft tissue volume, primarily in the labia majora, and to a lesser extent the mons pubis. As fat volume decreases, these structures may appear flatter, looser or more lax, creating a deflated appearance," Dr. Quintero explains.

GLP-1s Do Not Target Genital Tissue

Sean Simon, MD, a board-certified plastic surgeon in Miami, clarifies that GLP-1s do not specifically target genital tissues. Instead, rapid and substantial weight loss makes the concern more common and more noticeable.

"Significant weight loss affects fat stores throughout the body, including the face, breasts, abdomen, buttocks and genital region. Just as patients may notice 'Ozempic face' or volume loss in other areas, the vulva can undergo similar changes," says Dr. Quintero.

The anatomy explains the visible result. "The mons pubis (fatty mound over the pubic bone) and the labia majora (outer lips) naturally contain fatty tissue providing a plump, youthful volume and appearance. As the protective fat pads in the mons and labia majora shrink, the overlying skin (which may also lose elasticity during weight loss) may not contract fully, leading to a deflated, sunken or sagging appearance," explains Dr. Simon.

Prevention Strategies

Karen Horton, MD, a board-certified plastic surgeon in San Francisco, advises patients starting a GLP-1 to manage the rate of weight loss. "It's safest to lose weight slowly and gradually; this will facilitate skin contraction to your new form, rather than sudden deflation and hanging/loose skin," she says. Gradual loss also allows the skin's collagen and elastin to adapt to changing body composition.

"Pelvic floor exercises, such as Kegel exercises, can strengthen the underlying musculature and improve support, although they do not restore lost fat volume," notes Dr. Quintero.

Topical therapy can address tissue quality. "Topical treatments for women experiencing dryness or thinning of delicate tissues secondary to estrogen fluctuations (which may also accompany rapid fat loss), localized vaginal moisturizers or topical estrogen creams can help restore hydration and tissue thickness," says Dr. Simon. He adds that adequate daily protein intake and hydration support skin elasticity and overall tissue health as fat diminishes.

Treatment Options

Treatment plans vary by anatomy, goals, degree of volume loss and overall health. "The most successful approach is a comprehensive, individualized treatment plan that addresses both tissue quality and volume restoration," says Dr. Quintero.

For early laxity or volume loss, he recommends minimally invasive technologies that improve skin quality and collagen production. Tissue-tightening options include:

  • Fractional vaginal laser treatments
  • Radiofrequency microneedling, such as Morpheus8
  • Internal vaginal rejuvenation devices
  • Radiofrequency plasma technologies such as Renuvion or FaceTite in select patients

Volume restoration options include:

  • Autologous fat grafting (fat transfer)
  • Donor adipose allograft products, such as Alloclae, for patients without sufficient donor fat
  • Regenerative adjuncts including platelet-rich plasma (PRP) and platelet-derived growth factor (PDGF) therapies
  • Biostimulatory fillers

Dr. Quintero notes that tissue tightening and volume restoration can often be performed alongside labiaplasty, vaginal rejuvenation therapies, treatments for mild urinary incontinence, or other combined functional and aesthetic procedures. As GLP-1 prescriptions continue to expand the medical weight-loss market, surgeons who can address this under-recognized consequence are positioning themselves for a growing, largely unmet patient demand.
